With some insurance companies referrals are required when your primary care physician (PCP) refers you for a special test or visits with specialists.  In order to accommodate your referral request, we require 5 BUSINESS DAYS NOTICE TO PROCESS EACH REFERRAL.  If you are not sure that you need a referral, please contact your insurance carrier.  Please remember that it is your responsibility to know how your insurance works to meet you r medical needs.

Please follow these procedures:

Before you request a referral:

  1. Choose a specialist in your insurance provider’s network or inquire with your doctor or checkout staff, whom he recommends.  Be aware that it is your responsibility to make sure the specialist you are seeing takes your insurance.  A referral cannot be done for a doctor outside the insurance plan. 
  2. Call the specialist’s office and set up an appointment.

Requesting a referral:

  1. You can submit a referral request securely to us online by clicking on Health Forms. Create an account or log in with your existing account and fill out the Referral Request form.

  2. You can also request a referral by calling 410-252-CARE (2273).  

A Referral cannot be processed with out the following information:

  • Patient’s name, S-P-E-L-L-I-N-G the Last Name.
  • Daytime telephone number.
  • Patient’s date of birth
  • Full name of the specialist, S-P-E-L-L-I-N-G the full name.
  • Reason for the appointment (ex:initial visit, follow up, procedure, surgery). 
  • Diagnosis (what’s the purpose for the visit?) 
  • Date and time of the appointment.

A copy of your referral will be mailed to your home.

**Please note: AETNA REFERRALS ARE ELECTONRIC, which means they are on file with your insurance company immediately when entered and will not be faxed.  This also means that an Aetna referral cannot be backdated.

**We cannot do a referral for insurance information that we do not have on file!  So be sure your current insurance information is on file.
